Pre-Laser Treatment Medical Documentation
Our Medical Forms we collect basic medical information to use for to determine Go and No-Go laser treatment information that is H.I.P.A.A. compliant. We do not share any of this information. We document each laser treatment and store it in the patient’s laser treatment file.

We use a skin analysis to ensure a protocoled laser treatments every application of our three powerful and advanced Astanza F.D.A. approved lasers.
